How can you die from a shoulder/arm and rib fractures?

She wrote that she was disabled and she died in an assisted living facility, so maybe it had to do with her health being compromised anyway before the fractures? Like how an old person dies from a hip fracture?

If there are enough rib fractures (which lead to underlying lung injury) we can't always fix everything and get the person adequately re-oxygenated in time to avoid respiratory arrest and death. Also, any fracture of any bone (but more commonly the long bones) can lead to an uncommon but deadly complication called a fat embolism.
