When I was around 10, Outbreak made me have anxiety attacks and convinced me I was going to die of a horrible disease.
I can't think of anything else that's really disturbed me.
Candy Man is the only movie that's ever really scared me. I was also like 5 when I saw it.
The German Funny Games. I didn't see the American one even though Tim Roth is in it.

This was going to somewhat be my answer. The second real date with my bf was Irreversible in the theaters before it was pulled. This also is one of my favorites so far. Totally emotion provoking, when taken back you are left feeling so different than from start, middle, end. The flashing on the big screen was intense. Intense.

Pan's Labyrinth upset me. It was gorgeous, but being a mother has sapped me of my tolerance for portrayals of any kind of violence/emotional abuse towards children. Well, portrayals and real life. Whatever.

I remember watching the movie The Doors when I was like 7. I can't recall much detail about it now to say exactly what scenes that did for me but, it was a bit much considering my age. My dad took me to all Nightmare on Elm Street movies and I can honestly say he had his hands over my eyes more than not. Thanks Dad!

The American one is a shot by shot remake of the German one. It's by the same director. I didn't need to see that again, and my love for Tim Roth is strong.
These aren't horror films, but they affected me.
Storytelling was very disturbing to me.
Also, Ode to Billy Joe for some reason. :lol:
